Description: Advanced Practice Clinician - Nurse Practitioner/Physician Assistant

Program: ISNP Hours: 40 per week

Duration: Approximately 7 months

About The Role

The Senior Community Care (SCC) Program delivers longitudinal, integrated care for high-risk health plan members in facility-based settings. As an Advanced Practice Clinician (APC) within SCC, you will play a pivotal role in providing comprehensive clinical care while collaborating with an interdisciplinary team.

Key Responsibilities

• Perform comprehensive patient assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD-10 conditions
• Collaborate with an interdisciplinary care team
• Facilitate advanced illness and care planning discussions
• Provide education and counseling for patients and caregivers
  
  

Required Qualifications

• Active and unrestricted license in the state of residence (or ability to obtain by start date)

• Educational Background:
• Nurse Practitioner (NP): Master of Science in Nursing or Doctor of Nursing Practice
• Physician Assistant (PA): Graduate of an accredited PA degree program
• Certification:
• NP: Board-certified by AANP or ANCC in Family, Adult, Gerontology, or Acute Care
• PA: Board-certified by NCCPA
• Additional Requirements:
• Active DEA licensure/prescriptive authority
• Ability to perform physical tasks (e.g., lifting 30-pound medical bag, navigating various care environments)
• Reliable transportation
• Capability to secure a collaborative practice agreement (if applicable)
• Full COVID-19 vaccination (per federal, state, and local regulations)

Preferred Qualifications

• 1+ years of clinical experience (long-term care preferred)
• Experience managing complex behavioral, social, and functional patient needs
• Knowledge of geriatric care, chronic illness management, and end-of-life discussions
• Proficient with electronic medical records and documentation
  
  

Work Details

• Shift Hours: 8 AM – 5 PM (Monday to Friday)

• Dress Code: Medical attire (scrub color not specified)
• Travel: Required during shifts
• Coverage Area: Chatham, Guilford, Randolph
• Supporting another APC across 9 facilities, including:
• Alpine Health & Rehabilitation of Asheboro
• Siler City Center
• Universal Health Care/Ramseur
• Greenhaven Health & Rehabilitation Center
• The Shannon Gray Rehabilitation & Recovery Center
• Westchester Manor at Providence Place
• The Graybrier Nursing & Retirement Center
• Asheboro Rehabilitation & Healthcare Center
